5x sets of filled-in Diplomatic/Official Visa Application Forms (please click to download) for Indian Diplomats/Officials, their family members, and domestic servants of diplomats, signed in original with passport size photos pasted on the forms including a photocopy of the diplomatic/official passport are required for those being posted to the High Commission of India in Islamabad.
