Requirements for obtaining a manual passport for above 12 years of age
- Application Form-“A” (Please click to download) for New passport in lieu of Expired or Exhausted passport.
- Original old/expired passport.
- Original valid Pakistan National Identity Card along with a photocopy.
- Four current passport size (2X2, Front Face) latest photographs.
- Proof of residence in India (LTV/Any Indian Visa).
- Copy of request filed with Indian authorities for an extension of visa (if applicable).
- Affidavit stating that he/she has neither got any Pakistani passport after the present one nor he/she has been issued Indian Passport or any kind of documents of Indian citizenship.
